Oldfield Surgery Front

To register, a pack, which includes an introductory letter, purple GMS registration form, health questionnaire, SCR consent form, results consent form and patient information leaflets, Care date consent form, can be collected from the surgery.

When you attend the surgery with your completed forms you must bring with you Photo ID, proof of your address and your NHS number. Your NHS number can be obtained from your current GP surgery.

After your completed forms have been checked, you will then be registered with a named doctor, the name of which we are happy to give to you at any time, although you can choose to see any doctor at the practice.  Your registration will take 24 hours to go through our system, after which time you are welcome to book an appointment, also on registering we would like to invite you to make an appointment for a new patient medical with our practice nurse staff.

If you are on repeat medication it is advisable to book a doctors appointment to discuss this medication as soon as possible.
